| University Accessibility Center Accessible Book Creations (ABC) Alternate Format Policy and Procedures Policy Students with disabilities who require alternate access to printed material may receive their textbooks in an alternate format. Eligibility for this accommodation is determined on an individualized basis, and students must be able to demonstrate a substantial disability, which requires alternate materials. In order to ensure that your textbooks are available at the beginning of each semester, all relevant textbook requests must be forwarded to Accessible Book Creations (ABC) as soon as possible. Students are responsible for obtaining their textbook information as soon as they register for classes. If you require assistance in obtaining this information, you may contact an ABC employee by calling 801-422-4365 or through e-mail

. Typically, 6-8 weeks notice is required to obtain alternate formats. Late requests will result in a delay in providing materials. The ABC will make reasonable best efforts to provide materials as promptly as possible, and in the format requested by the student. However, consideration will be given to the most expedient manner to obtain materials. The following formats are available: · Text from the publisher (depending on availability) or text scanned by the ABC and given to the student, as either a text or audio file, provided to them on a CD · Recordings in DAISY format from Recordings for the Blind & Dyslexic (RFB&D) [http://www.rfbd.org], Library of Congress, National Library Service for the Blind and Physically Handicapped (NLS) [http://www.loc.gov/nls/], or live voice mp3s recorded by volunteers or ABC employees · Large print · Braille Proof of purchase (preferably the store receipt) for all of the materials requested on the form must be shown to the ABC Coordinator at the time when the alternative format is picked up from the office. If you decide to discard or sell your original textbook back to the bookstore, online, or to another student or individual, the alternative format CDs must be returned to our office. If you keep the textbooks or packets, you may keep the alternative format. Reasonable accommodations depend upon the nature and degree of severity of the documented disability. While the Americans with Disabilities Act of 1990 requires that priority consideration be given to the specific methods requested by the student, it does not imply that a particular accommodation must be granted if it is deemed not reasonable and other suitable techniques are available. Procedure · Qualify as having a disability as demonstrated through appropriate documentation and approved by your UAC advisor. · Determine which books you will require. This information can be obtained by calling the instructor of the course, the Bookstore textbook department (422-3003), or the department secretary for each course. When calling the Bookstore textbook department, please indicate that you are working with the University Accessibility Center. · If you have difficulty obtaining the information regarding your textbooks from the course instructor, course coordinator, academic department, or the BYU Bookstore, contact your UAC advisor immediately! This is important as requests are completed on a first-come, first-served basis. Therefore, the sooner the request is received by the ABC, the sooner the request will be completed. · Complete an Alternate Format Request Form available at the UAC - all lines on the form must be completed. It is strongly suggested that you get the request in at least six - eight weeks before classes start for the new semester. If this is not done the UAC cannot guarantee you will receive your request in a timely manner. · The Alternate Format Request Form is signed by your UAC advisor before it will be processed. · Take the request form to the ABC in room 1502 of the WSC where a Supervisor will go through it with you to ensure that all the necessary information has been included. (If you are out of town, you need to fax your request form to 801-422-0174, then immediately call the ABC to ensure that they have received your request form). · If you are requesting your texts in live voice the Supervisor will then check with the Learning Resource Center (LRC) in the HBLL, the Recording for the Blind and Dyslexic (RFBD), and Utah State Library (USL) for the required textbooks. If they have the textbooks, the Supervisor will order them (the UAC will not create alternative format text if it is already available). · The supervisor will then provide you with an estimated time frame for the converted text to be ready for collection. It is understood that the supervisor can make no guarantees of delivery, but will make reasonable best efforts to deliver them within the estimated time frame. · If you have not been notified that your converted textbooks are ready for collection by the end of the estimated date, it is important for you to immediately check with the ABC on the status of your books. You need to do this through e-mail at

. · If you no longer need a particular book or your classes have changed, you should contact the ABC by phone (422-4365) IMMEDIATELY so they can best accommodate these changes. It is to your advantage to have your class schedule finalized when you fill out the request form so that you will not have to make any new requests. Remember, all new requests will be processed by the date received in the ABC; therefore, the new request will be placed at the bottom of the list. · Once your textbook request has been processed, the ABC will inform you through e-mail when to pick up the completed request. · You are required to purchase any textbooks bought on your behalf for scanning at 75% of the original BYU Bookstore price before the UAC will distribute the digital version to you. The ABC will inform you which textbooks you are required to purchase from them. The textbooks must be paid for with the exact change or by check made payable to BYU. · You are responsible for returning ALL audio cassette tapes loaned to you in good condition. · Audio cassette tapes loaned to you are the property of Brigham Young University, USL, and RFBD. · ALL audiocassette tapes must be returned by Finals Week of that Semester. We will make reasonable efforts to contact you, but please do not wait for a call from our office. · If audiocassette tapes remain outstanding, or if you have not collected and paid for any textbooks purchased by the UAC on your behalf, the UAC will place a hold on your academic records. (See General Catalog, under “Registration,” the University withholds registration privileges, diplomas, and copies of academic records and transcripts, until all obligations are met.) · The CD/MP3/audiocassette tapes are protected by copyright©. Therefore, you MAY NOT copy, reproduce, sell, or place on the internet any of the alternative format texts, nor allow anyone else to do so. You will comply with all applicable copyright laws. · You are required to purchase a copy of each book you are requesting in an alternate format and are required to provide proof of purchase before materials will be provided. This can be done by either producing the actual textbook or the purchase receipt of the textbook(s). Once you are no longer the owner of the textbook (i.e. sold to the bookstore, online, student, or any individual, or discarded), you MUST return your alternative format CD to the ABC office. Any copies that were made onto any storage device must be destroyed.
